Felmerül a kérdés, hogy a szepszis mellett, maga a súlyos állapot, a sokk okozhatja-e a PCT emelkedését. Ebben, a 15 különböző sokkban szenvedő betegen végzett vizsgálatban azt találták, hogy szeptikus sokkban mindhárom marker szintje emelkedett volt, míg kardiogén sokkban a PCT nem emelkedett kórós szintig, ami azt sugallja, hogy pusztán a sokk nem jár fokozott PCT produkcióval. best predictive value for septic shock was found to be the measurements of NO2 sup -/NO3 sup - and procalcitonin concentrations. procalcitonin concentration represented the most suitable tests for defining patients with septic shock. In conclusion, the respective roles of infection and shock appear to be intricate and complementary in the stimulation of circulating proinflammatory cytokines, rendering the sensitivity and the specificity of the parameters we have measured rather weak for defining septic shock. Eisenberg és mtsai 48 szeptikus beteget vizsgáltak, és azt találták, hogy 14 ml/kg feletti tüdővíz esetén a mortalitás több mint duplájára nőtt. Az EVLW vezérelt folyadékterápia, szemben a pulmonális kapilláris éknyomás alapján végzett folyadékpótlással, szignifikánsan csökkentette a lélegeztetés, és az ITO kezelés hosszát. Gyenge korrelációt találtak viszont az EVLW és az oxigenizáció között.

Mortality risk increases for every day that procalcitonin increases. Levels or increases of C-reactive protein and white blood cell count do not seem to predict mortality.

A hemokultúra eredménye részint későn, 24 óra múlva, érkezik meg, továbbá gyakoriak az álnegatív, álpozitív eredmények. A citokineket illetően rövid féléletidejük, és a drága mérési tesztek miatt az ágymelleti diagnosztikában nem terjedtek el. Két marker maradt tehát versenyben, az évtizedek óta használt CRP, és a jóval fiatalabb PCT.
